2. Before First Use
- Inspect the packaging and instruments for shipping damage.
- Do not use instruments that are damaged, misaligned, corroded, cracked, or visibly defective.
- Clean and sterilize all instruments before first clinical use.
- Retain lot or batch information for traceability.
3. Cleaning After Use
Clean instruments as soon as practical after use. Do not allow blood, tissue, saline, or debris to dry on the instruments.
- Remove gross debris chairside using appropriate clinical protocol.
- Rinse instruments under lukewarm water.
- Keep hinged instruments in the open position during cleaning.
- Use a soft nylon brush to clean tips, serrations, hinges, joints, and textured surfaces.
- Use a neutral pH enzymatic or instrument detergent according to the detergent manufacturer’s instructions.
- Rinse thoroughly to remove all detergent residue.
4. Ultrasonic Cleaning
Ultrasonic cleaning may be used where permitted by local infection control protocols.
- Use an approved ultrasonic cleaning solution.
- Follow the ultrasonic cleaner and solution manufacturer’s instructions.
- Open hinged instruments during the cycle.
- Do not overload the ultrasonic basket.
- Rinse instruments thoroughly after ultrasonic cleaning.
5. Drying
Dry instruments completely before sterilization. Residual moisture may contribute to staining, corrosion, or reduced instrument life.
- Use a clean lint-free towel.
- Use medical-grade compressed air where appropriate.
- Ensure hinges, joints, serrations, and recessed areas are fully dry.
6. Inspection Before Sterilization
Inspect each instrument after cleaning and before sterilization.
- Confirm all visible debris has been removed.
- Check tips, beaks, hinges, and working ends for wear or damage.
- Check forceps and hinged instruments for smooth movement and proper alignment.
- Remove damaged instruments from service.
7. Sterilization
Steam sterilization is recommended. Always follow local infection control requirements and the sterilizer manufacturer’s instructions.
Common Steam Sterilization Cycles
- Pre-vacuum cycle: 132°C / 270°F for 4 minutes.
- Gravity cycle: 121°C / 250°F for 30 minutes.
Allow the full drying cycle to complete before storage.
- Use appropriate sterilization pouches, wraps, or cassettes.
- Do not stack instruments in a way that prevents steam contact.
- Keep hinged instruments open during sterilization unless local protocol differs.
- Do not use instruments until the sterilization cycle and drying cycle are complete.
8. Maintenance of Hinged Instruments
Hinged instruments such as forceps, hemostats, rongeurs, and needle holders require particular attention.
- Clean hinges thoroughly after use.
- Dry hinges completely before sterilization.
- Lubricate with medical instrument lubricant or instrument milk where appropriate.
- Do not use industrial oils or non-medical lubricants.
9. Storage
- Store instruments clean, dry, and protected from damage.
- Avoid prolonged exposure to moisture, saline, bleach, harsh chemicals, or acidic residues.
- Store instruments in a manner that protects tips, beaks, and working ends.
10. Warnings
- Do not use damaged, cracked, corroded, loose, or misaligned instruments.
- Do not exceed the intended clinical use of the instrument.
- Excessive force, improper leverage, or misuse may result in instrument damage or clinical injury.
- Always verify the instrument is appropriate for the intended procedure before use.
